Personal training online via FORMA LSR.
The 30-day challenge and the coaching program are services offered by David Sauceda, an individual doing business as Forma LSR (hereon referred to as "FORMA LSR"). The scope of the various services is stated in the description on the site https://go.forma-lsr.com/checkout-redirect-3-months. By registering and paying, you agree to these terms and will have access to the fitness app during your contract term.
Try the service
The 30-day challenge is completely free and will always be so. The 30-day challenge includes training programs that are not tailored. No coaching is included and no dietary advice is included. To get a more complete set-up with follow-up of PT, dietary advice and tailor-made programs, we recommend that you join our coaching program.
Age limit
You must be over 18 years of age to use our services, with the guardian's written consent and payment responsibility, so you can register if you have reached the age of 16.
Responsibility
FORMA LSR cannot be held responsible for any loss or damage caused by your login information falling into the wrong hands. However, you may be held responsible for any loss or damage that may be incurred by FORMA LSR if any unauthorized person uses your login information.
Medical liability and disclaimer
FORMA LSR or your PT has no medical knowledge; therefore, no medical advice is given. You are responsible for contacting a suitable healthcare provider if you have been ill or injured and are unsure if you can start training again. FORMA LSR is not responsible for any medical problems or injuries. FORMA LSR creates training and diet plans to help you reach or get closer to your fitness goals, but you are responsible for learning and ensuring the correct technique for all exercises.
Exercises are described via our fitness app through text and videos. We recommend hiring a local PT at a gym if you want to ensure you complete the exercises correctly. Under no circumstances is FORMA LSR responsible for any form of damage, direct or indirect, caused by your training, use, or inability to use our services. These disclaimers apply unless otherwise provided by mandatory legislation.
Health condition
Our services are not intended for individuals with acute injuries, nor for those with active eating disorders (such as anorexia, bulimia, or orthorexia). If you are pregnant, you are responsible for informing FORMA LSR immediately. If, as a result of your health condition, you are under the care of a physician or healthcare provider, you are responsible for discussing your diet and exercise plan with them before starting your program with FORMA LSR.
Guarantee and results
The diet and training recommended via FORMA LSR are methods that are scientifically grounded in research and experience. However, FORMA LSR makes no guarantee that you will achieve your specific goals or expectations, as results depend heavily on individual effort and adherence. We will always do our utmost to help you be successful. If you are dissatisfied with your diet or exercise schedule, please contact your PT in the chat; adjustments are usually remedied within 1 working day. In order for you to achieve your goals, it is vital that you follow the provided advice. Note that significant physical changes require a commitment to hard work over time regarding both exercise and diet.
Contract term period, training report and diet diary
The "Initial Term" of your membership is defined by the specific product or package purchased at checkout (e.g., 3 months, 6 months, etc.). You cannot cancel your subscription during this mandatory binding period. Your membership automatically rolls into a month-to-month plan following the Initial Term. A 30-day written notice is required for all cancellations. If notice is provided less than 30 days before your next scheduled billing date, one final charge will occur to satisfy the notice period, and app access will remain active until the end of that final paid month. Pause during the contract period
You can pause your training period in case of injury or long-term illness. In these cases, the client must provide a valid medical certificate via our https://go.forma-lsr.com/membership-management-form-6928. Once the certificate has been verified, the subscription can be paused for a maximum of one month.
Right of withdrawal
The right to cancel your registration or receive a refund only applies if the service has not yet begun. Once you have logged into the fitness app, or once your PT has begun work on your customized training or diet programs, the service is considered "in use," and the right of withdrawal is waived.
Use of the Services
The services may only be used for private use. You may not use your app or the services offered via FORMA LSR in a way that causes FORMA LSR or others to suffer damage or inconvenience. You may not violate the laws of your country, and by registering, you undertake to follow these rules.
Intellectual property rights
All material on our websites and within our fitness app is covered by the Copyright Act. All intellectual property rights, copyrights, and trademarks related to us, the coaching programs, and all digital content are held by FORMA LSR. It is not permitted to copy, publish, or otherwise distribute, transfer, or sell information, software, products, or services that form part of FORMA LSR.
Breach of contract
FORMA LSR has the right to terminate the contract period earlier without prior notice due to breach of contract, if you violate any point in these general terms or if FORMA LSR has reason to assume that this is the case. It is acceptable to train with a friend when using our services; however, account sharing is strictly prohibited. You are also welcome to mention your collaboration with us via social media.
Changes
FORMA LSR has the right to change the content of the website and app at any time without prior notice. FORMA LSR also reserves the right to terminate your membership at any time after your initial contract term period has been completed.
Personal data
You are responsible for providing true and accurate information about yourself, such as your first and last name. If you change your email address, you are responsible for notifying FORMA LSR. By registering, you agree that FORMA LSR may use your personal information to send information or coaching updates to you. Your personal data is not passed on to third parties and is handled in accordance with the General Data Protection Regulation (GDPR).
Choice of law and dispute
Any dispute relating to these terms or the use of the services offered by FORMA LSR shall be governed by the laws of the State of Texas, USA. Disputes shall be decided by the General Complaints Board or a court of general jurisdiction, to the extent that the board or court is competent to try the dispute.
